YES! I'd like to save money on my prescription
for Loestrin® 24 Fe—the birth control pill with a 3-day period.
I agree to participate in the Prescription Savings
Program. I am a resident of the United States and am at least 18
years of age or older. I understand that my response will be collected
and used by Warner Chilcott (US), LLC ("WC") and its agents to administer
the Prescription Savings Program. I understand that WC and its
affiliates may consolidate and use personal information that
individuals share with them in order to enhance the quality of
services that they offer or for other Loestrin® 24 Fe promotional
programs. The information that I provide will be used only by, or on
behalf of, WC and will not be sold or released to any other parties.
If I do not wish to participate in any additional programs I will
opt-out below or will contact WC at optout to
have my name removed from their program.
Important
Safety Information about Loestrin® 24 Fe: Oral contraceptives are not for
everybody. Most side effects of the Pill are not serious and those that
are, occur infrequently. Serious risks, which can be life threatening,
include blood clots, stroke, and heart attacks, and are increased if you
smoke cigarettes. Cigarette
smoking increases the risk of serious cardiovascular side effects,
especially if you're over 35. Women who use oral contraceptives should
not smoke. Some women should not use the Pill, including women who have
or have had blood clots, certain cancers, a history of heart attack or
stroke, as well as those who are or may be pregnant. The Pill does not
protect against HIV or sexually transmitted diseases.
